The High Needs ACO track launched under the Global and Professional Direct Contacting model during the Trump Administration and continued today as ACO REACH, provide high quality, coordinated care to patients with multiple chronic conditions, functional impairments, and significant social needs. These patients, while making up a relatively small percentage of the Medicare population, account for a disproportionate share of Medicare spending. These models have demonstrated the ability to improve care and lower costs, but policymakers must act to ensure that these patients have access to these models beyond 2026.
